Insurance companies and Wisconsin law require that key pieces of information are shared with you, obtained from you, and that this is all done before treatment begins. The following paperwork is relevant for you as an adult client (download all the "adult" links for a total of four, two-page forms) or if you are a parent of a minor (download all "child" links for a total of four, two-page forms).
